The successful candidate will provide comprehensive, continuous care to individuals and families across all age groups, while supporting high standards of clinical governance and service delivery. Key responsibilities: Deliver high-quality primary and preventive care, including diagnosis, treatment, health promotion, and chronic disease management. Conduct consultations for acute and long-term conditions and coordinate appropriate referrals. Lead multidisciplinary care planning and collaborate with nursing, specialist, and allied health teams. Maintain accurate, timely clinical records in line with professional and organisational requirements. Support clinical audits, quality-improvement initiatives, teaching, and mentoring where required.
