Educate individuals, families, and community groups on hygiene, nutrition, maternal and child health, immunization, family planning, and healthy lifestyle practices. Identify community health needs and provide appropriate health education, guidance, and referrals. Organize and participate in medical outreaches, health campaigns, wellness screenings, and vaccination programs. Perform basic health assessments, including blood pressure, blood glucose, temperature, weight, and other routine health checks where applicable. Support clients in accessing healthcare services by coordinating referrals and follow-up care. Conduct home visits to monitor clients' health status and reinforce health education: where necessary. Maintain accurate records of patient interactions, outreach activities, referrals, and community health reports. Collaborate with healthcare professionals and other stakeholders to ensure continuity and quality of care. Encourage adherence to prescribed treatments and promote positive health behaviors.
