We are recruiting to fill the position below: Job Title: Community Health Worker Location: Lagos Employment Type: Full-time Job Summary The successful candidate will play a key role in promoting health education, disease prevention, and community wellness by providing basic health services, conducting outreach activities, and connecting individuals with appropriate healthcare resources. We are seeking a compassionate, proactive, and community-focused Community Health Worker to join our healthcare team. The ideal candidate should possess excellent communication skills, a passion for community service, and a commitment to improving health outcomes. Key Responsibilities: Organize and participate in medical outreaches, health campaigns, wellness screenings, and vaccination programs. Identify and promptly report disease outbreaks, public health concerns, and emergencies to the appropriate authorities. Perform basic health assessments, including blood pressure, blood glucose, temperature, weight, and other routine health checks where applicable. Comply with organizational policies, infection prevention protocols, and health and safety regulations. Encourage adherence to prescribed treatments and promote positive health behaviors. Participate in health education: seminars, training sessions, and continuous professional development activities. Support clients in accessing healthcare services by coordinating referrals and follow-up care. Perform other related duties as assigned by management. Conduct community outreach programs to promote health awareness and preventive healthcare practices. Ensure confidentiality and privacy of patient and community information at all times. Identify community health needs and provide appropriate health education, guidance, and referrals. Educate individuals, families, and community groups on hygiene, nutrition, maternal and child health, immunization, family planning, and healthy lifestyle practices. Maintain accurate records of patient interactions, outreach activities, referrals, and community health reports. Collaborate with healthcare professionals and other stakeholders to ensure continuity and quality of care. Conduct home visits to monitor clients' health status and reinforce health education: where necessary. Requirements: Minimum of OND, HND, or Bachelor's Degree in Community Health, Public Health, Nursing, Health Education, Environmental Health, or a related field. Good understanding of primary healthcare principles and public health practices. 1–3 years of relevant experience: in community health, primary healthcare, public health, or wellness programs. Ability to communicate effectively in English and local languages is an added advantage. Proficiency in the use of Microsoft Office applications is an added advantage. Strong organizational, documentation, and reporting skills. Valid professional certification or license (where applicable) is an added advantage. Ability to work independently and collaboratively within a multidisciplinary team. Excellent communication, interpersonal, and counselling skills.
