Linking people to health care/social service resources. Providing informal counseling, support and follow-up as needed with patients. Requirements Candidates should possess an HND, NCE, OND, SSCE / GCE / NECO in relevant discipline. Experience working with people of different cultures and socio-economic backgrounds. Understanding of the healthcare system and how to help patients find necessary resources.
