Job Summary
They will conduct quarterly QoC assessment across supported facilities using the 8 standards of car.
The mentor will support facilities to identify quality gaps through data-driven approaches, strengthen adherence to national clinical standards and guidelines, institutionalize quality improvement processes, and enhance service delivery outcomes.
The RMNCAEH Quality of Care (QoC) Mentor will provide routine on-site mentoring, coaching, and technical assistance to health facilities to strengthen the quality of Reproductive, Maternal, Newborn, Child, Adolescent Health and Nutrition (RMNCAH+N) services.
Goal
To improve the quality of RMNCAEH and Nutrition services through continuous mentoring, coaching, quality improvement support, and data-driven decision-making at supported health facilities.
Specific Objectives
Improve the use of service delivery data for identification and resolution of quality gaps.
Establish, revitalize, and strengthen functional Quality Improvement Teams (QITs).
Improve referral and feedback mechanisms for maternal, newborn, child, adolescent health, and nutrition services.
Strengthen Infection Prevention and Control (IPC) practices within supported facilities.
Institutionalize continuous quality improvement using recognized QI methodologies and tools.
Strengthen health worker competencies in the delivery of high-impact RMNCAEH and Nutrition interventions.
Key Responsibilities
- Routine Mentoring and Coaching of health care workers
- The Mentor Will:
- Conduct regular facility visits (20 days per month) to provide on-site mentoring and coaching to healthcare providers.
- Data-Driven Quality Improvement Support
- Observe service delivery practices and provide constructive feedback for performance improvement.
- Facilitate skills transfer through demonstration, bedside coaching, and case reviews.
- Support providers to effectively implement high-impact interventions in maternal, newborn, child, and adolescent health, including nutrition services
- Reinforce compliance with national clinical protocols, standards, and guidelines.
- The Mentor Will:
- Track priority quality indicators and support facilities to improve performance.
- Review facility performance data to identify service delivery and quality improvement gaps.
- Quality Improvement (QI) Processes
- Guide facility teams to use data for evidence-based decision-making and planning.
- Support health workers to routinely analyze data from registers, patient records, tally sheets, DHIS2 reports, partographs, maternity registers, child health registers, nutrition registers, and other source documents.
- The Mentor Will:
- Strengthening Quality Improvement Teams (QITs)
- Support facilities to identify priority quality problems using routine service delivery data.
- Monitor progress of improvement projects and document lessons learned and best practices.
- Support facility teams to prioritize identified causes and develop change ideas.
- Facilitate root cause analysis using quality improvement tools including:Five Whys Analysis, Fishbone (Ishikawa) Diagram and Process Mapping
- Guide facility teams in the application of the Plan-Do-Study-Act (PDSA) cycle to test and implement solutions.
- The Mentor Will:
- Support QITs to conduct monthly meetings and maintain meeting records.
- Support facilities to inaugurate or revitalize Quality Improvement Teams (QITs).
- Facilitate the development of facility-specific quality improvement plans.
- Review QIT action plans and monitor implementation of agreed actions.
- Maternal, Newborn, Child, Adolescent Health and Nutrition Support
- Promote multidisciplinary participation and ownership of quality improvement activities.
- Orient QIT members on their roles and responsibilities.
- The Mentor Will:
- Referral System Strengthening
- Promote adherence to standards for ANC, labor and delivery, postnatal care, newborn care, child health, adolescent health, and nutrition services.
- Support implementation of interventions aimed at reducing maternal, newborn, and child morbidity and mortality.
- Strengthen nutrition assessment, counseling, referral, and follow-up systems.
- Provide technical guidance on quality service delivery across RMNCAEH and Nutrition service areas.
- The Mentor Will:
- Review referral registers and referral outcomes during mentoring visits. Support facilities to identify and address referral bottlenecks affecting service quality and continuity of care.
- Ensure proper documentation of referrals and feedback mechanisms between referring and receiving facilities.
- Provide technical support to strengthen two-way referral systems.
- Infection Prevention and Control (IPC)
- The Mentor Will:
- Assess compliance with IPC standards and protocols.
- Promote availability and appropriate use of IPC commodities and personal protective equipment.
- Identify IPC gaps and support implementation of corrective actions.
- Provide practical coaching on hand hygiene, environmental cleaning, decontamination, sterilization, and waste management.
- Support facilities to establish and maintain proper healthcare waste segregation and disposal practices.
- Documentation, Reporting and Follow-Up
- The Mentor Will:
- Develop mentoring visit reports with documented observations, recommendations, and action points.
- Highlight best practices, success stories, lessons learned, and persistent challenges requiring escalation.
- Maintain records of mentorship sessions, QI activities, and facility performance improvements.
- Monitor implementation of agreed action plans during subsequent visits.
- Share feedback with facility management and relevant stakeholders.
- Deliverables
- The Mentor Shall Produce:
- Monthly facility mentorship reports.
- Root cause analysis reports for identified quality gap and improvement tracking records.
- Quarterly QoC Assessment Report
- Facility-specific quality improvement projects.
- Monthly QIT meeting reports and attendance records.
- Required Qualifications
- The RMNCAEH Quality of Care (QoC) Mentor shall possess the following minimum qualifications, professional experience, technical competencies, and personal attributes:
- Educational Qualifications:
- Valid professional registration and current practicing licence with the relevant regulatory body, where applicable.
- Bachelor’s Degree in nursing, Midwifery, Medicine, Public Health, Community Health, or another relevant health-related discipline from a recognized institution.
- A master’s degree in public health, Nursing, Midwifery, Health Services Management, or a related field will be an added advantage.
- Professional
Experience
applying quality improvement methodologies, particularly Plan-Do-Study-Act (PDSA) cycles.
supporting health facilities to use routine health information and service delivery data for quality improvement and decision-making.
Demonstrated
working with primary healthcare facilities and health workers, preferably within Nigeria.
Demonstrated
using QI tools such as Five Whys, Fishbone/Ishikawa analysis, process mapping, and other root-cause analysis approaches.
Demonstrated
in establishing, strengthening, or supporting Quality Improvement Teams (QITs) and facility-level quality improvement initiatives.
At least 5 years of relevant professional
in RMNCAEH, primary healthcare, quality improvement, or health systems strengthening.
in Infection Prevention and Control (IPC), referral systems, clinical standards, and adherence to national guidelines.
conducting Quality of Care assessments using national or recognized QoC standards and assessment tools.
Technical Competencies
At least 3 years of demonstrated
providing facility-level mentoring, coaching, supportive supervision, or technical assistance to healthcare providers.
supporting the implementation of maternal, newborn, child, adolescent health and nutrition interventions.
Previous
working with State Ministries of Health, SPHCDA, LGAs, development partners, or other health-sector stakeholders will be an advantage.
The mentor should demonstrate sound knowledge and practical competence in:
Quality Improvement methodologies and tools.
Facility-level QoC assessment and performance improvement.
National clinical standards, guidelines, protocols, and quality-of-care requirements.
DHIS2 and other routine health information.
RMNCAEH and Nutrition service delivery at primary and secondary healthcare levels.
